Our ways of doing things

What we do and how we support our communities
.......

Smooth transitions to more independent living

We know that the move to a new service can feel like a big step, which is why we carefully plan each transition so that it is completely tailored to the individual’s needs. We are focussed on ensuring that the move to our services is a smooth and positive experience for each service user .

For some service users and family this means a gradual transition over a number of months to minimise any anxiety and ensure that they are looking forward to the journey ahead. Throughout the process our friendly and dedicated staff team offer support, advice and encouragement.

Process of Transition

  • Initiation of contact
  • Welcome Pack
  • Involving parties/care team meetings
  • Service need assessment
  • Risk assessment
  • Care Planning
  • Service watch/service review
  • Client voice/feedback
The Service User

We see the people using health and social services as equal partners in planning, developing and monitoring care to make sure it meets their needs. This means putting people and their families at the centre of decisions and seeing them as experts, working alongside professionals to get the best outcome. We consider our service users first.

Our Staff

We view our staff as the ambassadors of our services. It is therefore important that we have a robust recruitment and selection system which stands up to scrutiny. Employee training and development refers to the continued efforts of our company to boost the performance of our employees. We aim to continue to train and develop our employees.

Civility Care

Looking after the most vulnerable people in society is a pivotal role in health and social care, maximising their quality of life in the face of illness and disability. We as a provider will remain passionate, open and respectfully. We will continue to learn and re-relearn to better serve our service users and staff.
