A Smooth and Person-Centred Transition Process

We understand that moving into a new service or care arrangement can be challenging. That is why we follow a structured, six-stage transition model that ensures clarity, comfort, and confidence at every stage such as Initial contact and introduction, welcome pack and orientation, involvement of multidisciplinary teams, service needs assessment, risk assessment and mitigation planning, personalized care plan, Implementation and continuous monitoring, feedback. We adapt the transition pace depending on the individual—sometimes gradually over months—to reduce anxiety and promote positive engagement.
